*Please note that all verification requests regarding physicians who have completed residency more than 5 years ago, will incur a $60 verification fee. We accept checks, money orders and credit cards as forms of payment. Payments should be made out to the Washington University School of Medicine and sent to the address above. Credit Card transactions will incur an additional $5 convenience fee.
*Please allow 2 to 4 weeks for processing. If you require overnight service, there will be an additional $20 fee.
